Duties Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans aligned with clinical documentation standards. Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs to facilitate patient rehabilitation across orthopedic, neurological, pediatrics, and geriatrics specialties. Monitor patient progress regularly, adjusting treatment strategies as needed to maximize recovery outcomes. Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care plans and discharge planning efforts. Maintain accurate and detailed electronic health records (EHR) and electronic medical records (EMR) systems, ensuring compliance with documentation standards. Educate patients and their families on home & community care strategies to support ongoing recovery outside clinical settings. Assist with discharge planning by evaluating patient needs for continued therapy or community-based services, ensuring seamless transitions from inpatient or hospital environments to outpatient or home health care. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Proven experience working with outpatient rehabilitation centers, hospitals, or inpatient settings with a focus on outpatient clinic experience. Skilled in developing comprehensive care plans that incorporate patient goals and clinical best practices for treatment plan development. Excellent client communication skills to effectively educate patients about their treatment options and progress. Familiarity with outpatient experience including outpatient rehabilitation center operations and home health environments. Ability to work collaboratively with healthcare professionals across disciplines for integrated patient care. Proficiency in managing electronic health records (EHR) systems and EMR systems while adhering to clinical documentation standards. Experience working with diverse populations such as geriatrics, pediatrics, orthopedics, neurology, and patients requiring trigger point therapy or physiotherapy interventions. Knowledge of discharge planning processes and acute care experience enhances your ability to deliver comprehensive patient care.
