Up to $600.00 per day Job description: Home Health Physical Therapist/PTA Job Overview Prime Path Home Health is seeking a compassionate, motivated, and independent Physical Therapist to join our growing team serving Richmond and surrounding area. This position provides skilled physical therapy services to patients in their homes, helping them improve mobility, maximize independence, prevent falls, and achieve their rehabilitation goals. The ideal candidate is comfortable working autonomously, possesses strong clinical decision-making skills, and values building meaningful relationships with patients and families. This role offers scheduling flexibility, competitive compensation, and the opportunity to make a direct impact on the lives of individuals throughout the community. Duties Perform comprehensive physical therapy evaluations in the home setting. Develop individualized treatment plans based on patient goals, diagnoses, and functional limitations. Provide skilled interventions including therapeutic exercise, balance training, gait training, transfer training, neuromuscular re-education, and patient/caregiver education. Assess home safety and recommend modifications, adaptive equipment, and fall prevention strategies. Monitor patient progress and modify treatment plans as appropriate. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team. Complete accurate and timely electronic documentation in compliance with Medicare, insurance, and agency requirements. Participate in care coordination and interdisciplinary communication to ensure optimal patient outcomes. Educate patients and caregivers on disease management, safety awareness, and strategies to maximize independence. Maintain professional standards and adhere to all applicable state and federal regulations. Experience & Qualifications Graduate of an accredited Physical Therapy program. Current Virginia Physical Therapist license in good standing. Valid driver's license, reliable transportation, and automobile insurance. Strong communication, organization, and time-management skills. Ability to work independently and manage a flexible caseload. Home health experience preferred but not required. Experience with geriatric rehabilitation, neurological conditions, orthopedic diagnoses, balance disorders, and fall prevention is preferred.
