Job Summary Berlin Health & Rehab Center is a resident-centered and quality-focused skilled nursing and rehabilitation facility committed to creating a positive and supportive workplace. We are seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will provide physical therapy treatments under the supervision of a licensed Physical Therapist, helping patients improve their mobility, independence, and overall functioning. Responsibilities Implement individualized treatment plans established by the supervising Physical Therapist. Provide physical therapy treatments designed to improve patients' mobility, strength, function, and independence. Work collaboratively with other members of the rehabilitation team to implement programs based on each patient's needs and capabilities. Assist the Physical Therapist with tests, evaluations, and complex procedures as directed. Instruct patients, families, and caregivers in appropriate physical therapy techniques and exercises under the supervision of the Physical Therapist. Monitor patient response to treatment and communicate relevant observations to the Physical Therapist. Maintain accurate and timely documentation in accordance with facility and regulatory requirements. Promote a safe, supportive, and patient-centered rehabilitation environment. Qualifications Graduate of an accredited Physical Therapist Assistant program recognized by the American Physical Therapy Association (APTA). Must be licensed or eligible for licensure as required by the state of practice. Strong organizational and time-management skills. Excellent verbal and written communication skills. Ability to work effectively as part of a collaborative rehabilitation team. Compassionate, dependable, and committed to providing high-quality patient care.
