Physical Therapy Assistant (PTA)

We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our rehabilitation team in Fort Atkinson, WI. The PTA will work under the supervision of a licensed Physical Therapist to provide high-quality therapy services to residents in our skilled nursing facility. This role supports residents in restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities.

Responsibilities:

Provide physical therapy treatments and interventions as directed by the supervising Physical Therapist. Assist residents in improving strength, mobility, balance, and coordination to achieve individualized care goals. Instruct residents in safe and effective exercises, mobility techniques, and the use of adaptive equipment. Monitor residents' progress and communicate changes or concerns to the Physical Therapist. Document therapy sessions, resident responses, and progress accurately and timely in medical records. Collaborate with the interdisciplinary care team to develop and update individualized care plans. Educate residents, families, and caregivers on therapy techniques to promote continued progress outside of therapy sessions. Ensure therapy equipment is safe, clean, and properly maintained. Adhere to all federal, state, and facility regulations and standards of care.

Qualifications:

Graduate of an accredited Physical Therapist Assistant program. Current licensure as a Physical Therapist Assistant (PTA) in the state of Wisconsin. Prior experience in a skilled nursing facility or similar healthcare setting preferred, but not required. Strong communication and interpersonal skills with the ability to motivate and encourage residents. Compassionate, patient, and resident-centered approach to care. Ability to work effectively both independently and as part of a healthcare team.
