Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or Physician Assistant with at least 2 years of experience, preferably in long-term care. They will independently manage patients' comprehensive medical needs in PALTC settings, focusing on improving care quality, patient outcomes, and reducing hospitalizations and costs. Responsibilities include conducting assessments, diagnosing, treating, prescribing, and developing care plans, collaborating with facility teams, ensuring compliance, and participating in safety protocols. The role requires strong communication, clinical decision-making, and proficiency with EHR and telemedicine tools. Qualifications include a relevant degree, board certification, and licensure as an APRN or PA.
