Advanced Care at Home - Locum Tenens Acute Care Nurse Practitioner or Physician Assistant - Walnut Creek, CA GHR Healthcare is partnered with a major healthcare system seeking an Advanced Practice Provider to support their 'Advanced Care at Home' program on a long-term, full-time contract. You ll represent an innovative care model that brings advanced clinical expertise beyond traditional walls and into the communities we serve. APPs work seamlessly within a multimodal, multidisciplinary care team collaborating across nursing, physicians, pharmacy, therapy, care coordination, and virtual health partners for patients who can be cared for safely in their homes. Schedule:

10-hr shifts, 7 days on 7 days off - 8a-6p

PST Territory:
