A Physician Assistant provides medical care to patients, under the supervision of a physician or surgeon. Works in all areas and specialties of medicine, and is licensed to practice medicine but has less formal certification than a physician. Performs duties delegated by a physician such as recording patient medical histories, ordering diagnostic tests, or giving immunizations. May serve as the primary healthcare provider, in consultation with a part-time physician, in communities with limited health care resources.

Bring the Exam Room to Your Patient Your practice won't be limited to one building. You'll round throughout assisted living, memory care, independent living, and skilled nursing communities across Southern Minnesota. One visit might involve routine management of diabetes, CHF, COPD, or hypertension. The next could involve evaluating an acute change in condition. Another may be a resident who recently returned from the hospital and needs someone to reconnect the pieces of their care . You'll assess the patient, make clinical decisions, communicate with the care team, and determine what needs to happen next. And because you're caring for patients where they live, you'll have access to something incredibly valuable: the people who know them best . What You'll Be Doing Provide comprehensive primary care throughout senior living and long-term care communities . Travel throughout Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities . Evaluate, diagnose, and treat acute and chronic medical conditions . Perform routine follow-ups, assessments, and preventative care. Develop individualized treatment plans. Manage chronic diseases and medically complex older adults . Recognize changes in condition and intervene proactively. Coordinate care following hospitalizations, rehabilitation stays, and specialist visits. Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and senior living teams . Educate patients and families regarding diagnoses, medications, and treatment plans. Help reduce avoidable emergency department visits and hospitalizations . Maintain accurate and timely clinical documentation. What We're Looking For Required Qualifications Graduate of an accredited Physician Assistant program. Active Minnesota PA-C license in good standing. Excellent assessment, diagnostic, and clinical decision-making skills . Ability to practice autonomously while collaborating within a physician-led team. Confidence managing multiple chronic conditions. Strong communication and care coordination skills. Genuine interest in caring for older adults. Reliable transportation and willingness to travel throughout Southern Minnesota . Preferred Qualifications Experience serving seniors or medically complex patients . Chronic disease management expertise. Community-based healthcare experience. Assisted living, memory care, skilled nursing, or long-term care experience. Advanced wound care experience.
