Job Description

Job Summary The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant to 'float' to various skilled nursing facilities in northeast Pennsylvania. Job Duties This position is as a Geisinger provider in a non-Geisinger facility and requires an experienced advanced practitioner (AP), prior experience working in a skilled nursing facility, family medicine, internal medicine or ICU. The AP provides direct (hands-on) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting. This position has a $10,400 float differential and will travel to these locations, but are not limited to: Mountain Top Rehabilitation & Healthcare Center, Mountain Top Birchwood Rehabilitation and Healthcare Center, Nanticoke Lakewood Rehabilitation & Healthcare Center, Nanticoke The Meadows, Dallas Embassy of Wyoming Valley, Wilkes Barre, Embassy of Scranton, Scranton Oak Ridge Rehabilitation and Healthcare Center, Scranton Geisinger has much to offer: $10,400 float differential $15k recruitment incentive Attractive schedule

Responsibilities:
Review reports Communicate with SNF staff for patient needs Treat patients for acute needs, admissions, discharges, etc. Manage extremely complex patients, see every day or at least once a week Approximately 12 patients a day Length of time per patient and number of patients is per need Position Details Education Master's Degree-Physician Assistant (Required) Experience Certification(s) and License(s) Certified Physician Assistant
