Job Description Centra's PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that's most appropriate for each participant-whether that's in their own home, with family, in long-term care, or assisted living. PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff. Centra's PACE program is seeking a dedicated Advanced Practice Provider to join our team and help shape the future of our care model. Responsibilities You Can Expect:

Monday

• Friday 8:00am
• 5:00pm
• no weekends or holidays! Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers 5 patients per day
• small volume Call is once every 7 weeks, 5pm
• 8am, you are not expected to go out, calls are triaged before reaching you!

Qualifications Required Education:

Masters Degree from an accredited Physician Assistant School 2 years of Physician Assistant experience required

Required Certifications and Licensures:

Current licensure by the State of Virginia as a Physician Assistant ACLS (required only for LFM)
