Job Description Summary The Primary Care Physician (PCP) at CareMore Health delivers high-quality, patient-centered care in a CareMore clinic and associated care settings, supporting CareMore's mission of improving health outcomes through value-based care, preventive medicine, and integrated care team collaboration. This role provides comprehensive primary care services to a complex patient population while driving clinical quality, patient experience, and stewardship of total cost of care. How will you make an impact & Requirements Key Responsibilities Clinical Care & Patient Outcomes Provide direct patient care including interviewing, examining, diagnosing, and treating patients across a broad range of primary care needs. Deliver evidence-based medical care with a strong emphasis on prevention, early detection, chronic disease management, and care gap closure. Manage patients with complex clinical needs, including multiple comorbidities and high-risk conditions, with a focus on measurable outcomes. Order, interpret, and act upon diagnostic tests, imaging, and labs; coordinate specialty referrals when clinically appropriate. Support appropriate care coordination and follow-up planning to reduce avoidable ED visits and hospitalizations. Integrated Team-Based Care Collaborate with an interdisciplinary care team that may include Nurse Practitioners, Physician Assistants, Medical Assistants, Care Managers, Social Workers, Pharmacists, and other clinical support staff. Participate in collaborative care planning, including escalation pathways and care transitions support. Communicate effectively with internal and external partners to ensure continuity of care. Quality, Documentation & Compliance Complete accurate, thorough, and timely documentation in the electronic medical record (EMR) in accordance with CareMore standards. Ensure alignment with clinical quality programs, regulatory requirements, and established policies and procedures. Maintain strict confidentiality of patient information in compliance with HIPAA and other federal/state regulations. Support timely and professional resolution of patient concerns, complaints, and inquiries. Performance Improvement & Patient Experience Participate in quality improvement initiatives, clinical pathway adherence, clinical management programs, and continuing medical education. Support patient experience efforts through compassionate communication, clear education, and proactive engagement. Educate patients on wellness, disease prevention, and early detection strategies. Contribute to a culture of safety, accountability, and continuous improvement to enhance outcomes and service delivery. Qualifications (Required) MD or DO degree required Current, unrestricted medical license in the applicable state(s) Board Certified or Board Eligible in Internal Medicine or Family Medicine Current DEA registration (as applicable to state practice requirements) Satisfactory completion of required health screenings (including Tuberculosis testing must be within the last 12 months) BLS Certification Hep B Vaccinations (all 3 doses, titer or signed declination)

$211,369.00 to $317,053.00
