Job Description
About the Role We are seeking a Full-Time Functional Medicine Nurse Practitioner or Physician Assistant with a strong interest in health optimization, wellness, and longevity. This role is ideal for an advanced provider who combines clinical excellence with a passion for functional medicine and a desire to make a meaningful impact on patients' lives. You will work as part of a collaborative, multidisciplinary team, helping to deliver personalized, root-cause-based care while contributing to the growth and innovation of a forward-thinking healthcare practice. Key Responsibilities
- Conduct comprehensive patient assessments, including diagnosis, treatment, and follow-up care
- Evaluate, diagnose, and recommend individualized treatment plans and wellness protocols
- Perform and manage hormone replacement therapy (HRT)
- Order, interpret, and analyze lab work, including blood panels and advanced diagnostics
- Develop personalized care plans based on patient history, lifestyle, and health goals
- Educate patients and their families on disease prevention, lifestyle optimization, and treatment plans
- Collaborate with physicians and other healthcare providers to ensure continuity of care
- Maintain accurate and thorough documentation in electronic medical records (EMRs)
- Participate in quality improvement initiatives and stay current with clinical best practices
- Contribute to a positive, team-oriented clinical environment Qualifications
- Master of Science in Nursing (MSN) or equivalent with APRN certification, or Physician Assistant certification
- Active state license to practice as a Nurse Practitioner or Physician Assistant
- Minimum of 3+ years of experience in primary care or a related field
- Minimum of 3+ years of experience in functional medicine
- Experience with hormone therapy preferred
- Strong clinical, diagnostic, and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively within a team environment
- Proficiency with EMRs and clinical software systems What We Offer

Pay:
$110,000.00- $155,000.
