Job Description

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

PACE Primary Care Physician Schedule:
Monday-Friday, 8am-5pm
Location:
Lafayette, CO Pay:
$229,270 to $283,888 per year
On Call:
On-Call around 5 days per month. 1 night per week Mon-Thurs 5p to 8a, and 1 weekend split either Friday 5p to Sun 8a, or 1 Sun 8a- Mon 8a.
JOB SUMMARY
The PCP works in collaboration with other providers and interdisciplinary teams to deliver primary care and other health services to geriatric participants enrolled in PACE. The provider may also participate in clinical call rotation for TRU Community Care and provide care to hospice patients.
ESSENTIAL DUTIES
Provides diagnosis, treatment and management services for PACE participants and hospice patients as directed. Participates actively in interdisciplinary team conferences, helping to develop and revise participant/patient care plans and acting as a medical resource in determining approaches to individual participant/patient care Responsive to provide urgent visits in the clinic or in the participant home when required during work day and when on call during the day or on the weekend Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others
EDUCATION AND QUALIFICATIONS
2+ years of primary care experience Unencumbered medical license in Colorado (MD, DO) Controlled substance license with current DEA license number Eligible for or currently possess a Medicare/Medicaid UPIN Eligible to bill insurance for palliative care and symptom management visits Experience working with frail/elderly (preferred) Must have $1M/$3M professional liability insurance and TRU will reimburse the premium costs Must possess a valid driver's license, provide proof of insurance (Colorado state minimums) and have reliable transportation Must comply with agency health screening requirements, including medical clearance for all communicable diseases prior to evidence of direct participant contact, and proof that all immunizations are up to date prior to direct participant contact #ZR TRU's Benefits (Eligibility Varies by Position/Status): Paid Vacation time 401(k) with match Health, dental, vision & life insurance
