The Primary Care Physician at PCHC provides comprehensive, patient-centered medical care to individuals and families across the lifespan within a community health center setting. The physician delivers preventive, chronic, and acute care services while supporting PCHC's mission to improve access to quality healthcare for underserved populations. The physician works collaboratively within PCHC's integrated care model alongside behavioral health, perinatal, dental, pharmacy, nursing, and care coordination teams to promote whole-person care and improve patient outcomes. 23) Practice Highlights Flexible weekday schedule with no weekends or holidays 30 patient contact hours with 10 hours of dedicated administrative time No call responsibilities or after-hours requirements Dedicated Urgent Care team and strong collegial cross-coverage Average patient volume of only 12-14 patients daily Patient panel size capped around 1,200 patients per provider Robust clinical support teams including RNs, dedicated MAs, case managers, referrals, and dietitians
EPIC EMR
system with Dragon and AI-assisted documentation tools Integrated services including on-site Pharmacy, Laboratory, Behavioral Health, Women's Health, Perinatal Care, and Dental Collaborative medical staff consisting of 20 Physicians and 40+ Advanced Practice Providers Mentorship and long-term career development opportunities available Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution. Board Certified or Board Eligible in Family Medicine or Internal Medicine. Active and unrestricted Colorado medical license or ability to obtain prior to start date. DEA certification required. Current BLS certification required. Preferred Qualifications Prior experience working in a Federally Qualified Health Center (FQHC), community health center, or underserved setting preferred. Bilingual skills preferred but not required. Experience with EPIC or comparable electronic medical record systems preferred. Knowledge, Skills, and Abilities Commitment to serving diverse and underserved patient populations. Strong interpersonal and communication skills. Ability to work effectively within a multidisciplinary team environment. Excellent clinical judgment and organizational skills. Understanding of value-based care and population health principles.
