Worksite Setting:
Outpatient Scope of Work:
General FM practices; annual wellness visits, well child visits, annual CPE visits, chronic disease follow up, acute care visits, transition of care management, in basket cross coverage Procedures in-office: joint injections, IUD insertions, implantable contraception, L&D of abscess and skin biopsies Prenatal care is optional Office located adjacent to urgent care center and OBGYN services are in same location 3 FM providers in practice
Licenses, Certifications, Requirements:
Active CT license preferred, IMLC accepted w/LOQ,
