Join a highly rated, physician-led group practice in the Danbury area. This is an excellent opportunity for a board-certified internal medicine physician to provide post-acute care services in a collaborative, flexible environment.

Responsibilities:
Deliver high-quality, cost-effective patient care in post-acute care communities Collaborate with interdisciplinary clinical teams while following CMS guidelines Partner with hospice, palliative care, and home care agencies to ensure safe patient transitions Maintain timely documentation and billing through EMR in compliance with Medicare standards

Fee-for-service (expected annual range: $255,608 $368,076, with no cap on productivity bonus potential)

Qualifications:
Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine Post-acute care experience preferred (willing to train experienced physicians from other care settings) Active Connecticut medical license (or ability to obtain)
