Job Description

(Physician/MD qualifications required) Internal Medicine - The Family Practice or Internal Medicine Physician will provide whole person healthcare in an integrated care setting. The Physician will diagnose and treat acute and chronic illnesses, provide routine health screenings and counseling on lifestyle changes to prevent illnesses before they develop. The Physician will guide and coordinate all aspects of care as an engaged and active member of a multi-disciplinary treatment team. The Physician will work with the team to achieve the best possible patient outcome in the most cost-effective manner. Appointments are scheduled and there are some walk-ins. Target is 20 patients per 8-hour day. Documentation is completed electronically concurrent in session in our EHR, NextGen. Other clinic staff include Medical Director, front desk, medical assistants, RNs, APRNs, pediatrician, dental staff and behavioral health staff.
Qualifications:
Doctor of Medicine or Doctor of Osteopathic Medicine with Board Certification in Family Practice or Internal Medicine required. Licensed physician in the State of Connecticut. Active DEA controlled substance registration and an active Connecticut controlled substance registration. Must have or be willing to obtain suboxone waiver.
