Primary Care Physician Sandbar Health Washington, DC Job Details Full-time $230,000 - $270,000 a year 4 hours ago Qualifications BC/BE Internal Medicine Outpatient clinic experience Urgent care Emergency medicine BC/BE Family Medicine Full Job Description Position Summary We are seeking a Board-Certified Family Medicine Physician to serve as a foundational clinical leader within a high-touch, technology-enabled primary care practice. Physicians will work in a collaborative POD-based care model alongside Advanced Practice Providers (APPs), health coaches, medical assistants, and behavioral health clinicians to deliver preventive, relationship-driven care with a strong focus on access, continuity, and proactive population health management. This role is ideal for a physician who values meaningful patient relationships, innovation in care delivery, and the opportunity to help shape a modern, consumer-oriented healthcare organization from the ground up. Core Responsibilities Deliver comprehensive outpatient Family Medicine care for adult and pediatric patients in a concierge-style environment Lead preventive care and longitudinal chronic disease management with an emphasis on reducing downstream utilization Provide same-day and next-day access through a combination of in-person, virtual, and asynchronous care modalities Collaborate with APPs, behavioral health clinicians, health coaches, and care navigators within a team-based POD model Perform office-based procedures and women's health services within scope of practice Participate in proactive outreach efforts focused on preventive health, care gaps, and high-risk patients Utilize digital tools, structured intake pathways, and AI-supported workflows to improve efficiency and patient experience Support clinical pathway development and standardization initiatives Contribute to a culture centered around hospitality, responsiveness, and patient-centered communication Assist in shaping clinical operations and future scaling of the organization Clinical Model The practice is designed around smaller patient panels and high-access care delivery. Each POD will support approximately 500-750 patients and include a physician, APP(s), medical assistant/health coach, and embedded behavioral health support.
The care model emphasizes:
