Heartline Staffing is recruiting a Primary Care Physician for an onsite opportunity in Deerfield Beach, Florida, focused on providing comprehensive primary care to complex Medicare and senior patients. This physician will work within a collaborative, team-based practice model with a small patient panel and resources designed to support high-quality, coordinated care.

POSITION OVERVIEW

This role provides direct patient care primarily in an ambulatory setting, with care occasionally extending into acute care, nursing homes, skilled nursing facilities and home settings depending on the assignment. The physician is responsible for comprehensive assessment, diagnosis, treatment, care planning, patient education, referrals, care coordination and clinical documentation.

Base Salary Range:

$238,832

•341,189 Position May Also Be Eligible For Bonuses Or Commissions

SCHEDULE
Monday-Friday Clinic Hours:

7:30 AM-5:00 PM Onsite Position

PRACTICE MODEL
Exclusive Focus On Complex Medicare Patients Small Patient Panel Physician-Led Practice Model Collaborative, Team-Based Care Onsite Pharmacy Services Onsite Diagnostic Services Shuttle Service Available For Patients ROLE Perform Comprehensive Patient Assessments And Medical Histories Conduct Physical Examinations And Evaluate Acute And Non-Acute Clinical Problems Develop, Document And Implement Patient Management Plans Diagnose And Treat Medical Conditions Order And Interpret Appropriate Laboratory And Diagnostic Studies Order Appropriate Medications And Treatments Coordinate Specialty Referrals And Case Management Referrals Provide Patient And Family Counseling, Education And Health Promotion Participate In Patient Care Rounds And Clinical Conferences Collaborate With Multidisciplinary Care Teams Develop And Communicate Patient Management Strategies Support Safe Transitions To Appropriate Levels Of Care Coordinate With Physicians, Nurses, Physical Therapists, Social Workers, Families And Caregivers Initiate Life-Saving Procedures When Clinically Necessary Support Quality, Patient Satisfaction And Readmission-Reduction Goals Maintain Clear, Detailed Clinical Documentation And Progress Notes Requirements:

Current Unrestricted Florida MD Or DO License

REQUIRED

Board Certification In Internal Medicine Or Family Medicine

Current DEA Number For Schedule II-V Controlled Substances

REQUIRED

Basic Cardiac Life Support Certification

REQUIRED

Ability To Perform Comprehensive Patient Assessments And Develop Management Plans

REQUIRED

Strong Communication Skills For Patient Education And Multidisciplinary Collaboration
