Responsibilities Diagnose and treat acute and chronic illnesses Manage conditions such as diabetes, hypertension, COPD, heart failure, and other chronic diseases Develop personalized treatment plans Perform annual wellness visits and preventive care Collaborate with specialists and interdisciplinary care teams Educate patients and families to promote healthier lifestyles Maintain timely and accurate EMR documentation Supervise Advanced Practice Providers when appropriate Qualifications MD or DO Board Certified or Board Eligible in Family Medicine or Internal Medicine Active Oklahoma license (or ability to obtain) Outpatient primary care experience preferred Passion for senior-focused, value-based healthcare Strong communication and leadership skills Who Thrives Here?
