Compensation:

Base $260,000-$290,000 + up to 20% of base salary in bonus Bonus based on: 1. Patient satisfaction rating 2. Living the companys values 3. HEDIS compliance 4. Reducing hospital sick days Primary Care Outpatient Value Based Care Model Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)
