The Primary Care Physician (PCP) at ChenMed is responsible for delivering high-quality care to a panel of up to 450 patients, focusing on accountability for outcomes and health coaching. The PCP will work collaboratively with care teams to achieve performance goals and ensure compliance with healthcare guidelines. This role requires a commitment to patient-centered care and the ability to lead and inspire others in a positive culture. The candidate will be responsible for providing care for patients with acute and chronic illnesses encountered in the older adult patient. The PCP will take ownership of patient care and outcomes, seeking consultation from specialists when necessary. The candidate will help drive innovation in patient health by acting as a health coach and assisting patients in setting health goals. The PCP will collaborate cross-functionally to simplify and prioritize care for patients, ensuring actionable behavior changes. The candidate will lead efforts to manage their care team effectively, ensuring high-quality patient care and compliance with guidelines. An MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required. A current, active State Medical License for the state(s) in which the candidate will be working is mandatory. The role requires proficiency in Microsoft Office Suite products including Word, Excel, PowerPoint, and Outlook. Experience with evidence-based medicine and a commitment to continuous improvement in patient care is essential.
