Perform assessments, manage chronic conditions, and coordinate specialist care. Provide pediatric and prenatal/postnatal care where applicable. Demonstrate strong communication, cultural sensitivity, and organizational skills. Require Illinois Medical license or eligibility, Board Certification or Eligibility, and MD or DO degree. Work Monday-Friday, 8:00am-5:00pm, with clinical and scholarly responsibilities. Benefits include health insurance, 401(k) match, paid time off, tuition reimbursement, and more. Salary ranges from $225,000 to $295,000 annually.
