Job Description:
We are actively seeking a Board Certified Family Medicine or Internal Medicine Physician for a Primary Care Medical Director opportunity in Mishawaka, Indiana within a structured PACE (Program of All-Inclusive Care for the Elderly) model. This is a permanent leadership position overseeing clinical operations and primary care delivery for a frail and elderly patient population.           Hours:
          8:00 AM 4:30 PM

          Clinical & Leadership Responsibilities:
          • Provide medical direction and oversight of primary care services
          • Supervise and collaborate with APPs and interdisciplinary team members
          • Deliver comprehensive care to frail/elderly patients
          • Conduct physical exams and medical assessments
          • Develop individualized care plans
          • Participate in quality improvement and program development initiatives
          • Ensure compliance with federal, state, and regulatory standards
          • Guide clinical protocols and standards of care This model focuses on keeping elderly patients living independently in the community through coordinated, preventative, and rehabilitative services.
          Patient Population:
          • Geriatric / frail elderly
          • Medicare/Medicaid population
          • Complex chronic conditions
          • Interdisciplinary care coordination
          Requirements:
          • MD or DO
          • Board Certified in Family Medicine or Internal Medicine
          • Active Indiana medical license (or ability to obtain)
          • Minimum 1 year experience working with frail or elderly populations
          • Leadership or supervisory experience preferred
          • Comfortable in collaborative, team-based care settings Visa sponsorship not available.
