Primary Care Provider - Kokomo, IN /
Jeffersonville, IN Job Type:
Full-Time Shifts:
Days (Monday-Friday); Shared on-call coverage (no weekend shift differential)Healthcare Setting:
Outpatient Senior Care /Community Health Responsibilities:
- Provide direct primary care services, including assessments, diagnoses, treatment, and medical record documentation
- Collaborate with interdisciplinary care teams to coordinate patient-centered care
- Offer medical consultations and support to clinical staff and care coordinators
- Participate in care planning meetings and contribute to clinic operations
- Deliver age-specific and geriatric care with a focus on safety and dignity
- Ensure compliance with clinical protocols, safety standards, and regulatory guidelines
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Attend staff meetings, trainings, and internal committees as required
Qualifications:
- MD/DO degree from an accredited medical school
- Board Certification in Family Medicine or Internal Medicine
- Licensed or eligible to practice in Indiana
- CPR certification preferred
- At least 2 years of experience working with frail or elderly populations in long-term care
- Experience in senior care or PACE programs is a plus
