Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. PACE Program Designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Physician Provides comprehensive primary and geriatric medical care to older adults, supporting their ability to remain healthy, independent, and engaged in their communities. As a core member of the interdisciplinary care team, the physician conducts medical assessments, diagnoses and manages acute and chronic conditions, develops individualized treatment plans, and oversees participants' overall healthcare needs. The role requires close collaboration with nurses, therapists, social workers, pharmacists, and caregivers to coordinate services across medical, behavioral, and social domains. Physicians also play a key leadership role in care planning, preventive health initiatives, medication management, and transitions of care, ensuring that participants receive integrated, person-centered services that enhance quality of life, optimize health outcomes, and support aging with dignity and independence. Location Based in Topeka, with travel to Lawrence and Emporia required. Required Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution Current, unrestricted medical license in Kansas (or eligibility for licensure) Board certification or board eligibility in Family Medicine, Internal Medicine, Geriatric Medicine, or a related primary care specialty. Experience providing primary care to older adults and managing multiple chronic conditions Strong clinical assessment, diagnostic, and care-planning skills Ability to work collaboratively within an interdisciplinary team environment Excellent communication, leadership, and documentation skills Commitment to person-centered, community-based care and the PACE model of care Preferred Board certification in Geriatric Medicine Previous experience in a PACE program or similar integrated care model Experience working with medically complex older adults, dementia care, and palliative care populations Leadership experience within interdisciplinary healthcare teams Familiarity with value-based care, Medicare and Medicaid programs, and population health management Experience in home-based, community-based, or long-term care settings Proficiency with electronic health records and quality improvement initiatives Strong communication and interpersonal skills What's in it for you?
