Job Description

Board-certified or board-eligible Internal Medicine (MD or DO) physician needed to join a facility-based post-acute/inpatient hospitalist team in Portland, ME. Full-time, Monday-Friday, 9:00 AM-5:00 PM schedule. No weekends and no call.
Locations:
Portland, Maine (primary) Additional opportunities: Hyannis, MA; Burlington, VT; St.
Albans, VT Schedule & Practice Model:
Monday-Friday, daytime hours (9 AM-5 PM) No weekends, no call Facility-based inpatient/post-acute care (skilled nursing/long-term care) Full-time, part-time, and PRN roles available High clinical autonomy with dedicated clinical support Focus on continuity and long-term patient relationships
Clinical Responsibilities:
Manage medically complex hospitalized and post-acute patients Coordinate care with nursing, therapy, case management, and families Perform admissions, daily management, discharge planning, and follow-up Document appropriately and communicate transitions of care
Compensation & Benefits:

Minimum Qualifications:
MD or DO from accredited medical school Board certified or board eligible in Internal Medicine; Family Medicine, Geriatrics, or related specialties considered Active, unrestricted state medical license or eligibility for licensure DEA registration or eligibility to obtain Experience in hospital medicine, post-acute care, skilled nursing, long-term care, geriatrics, or internal medicine preferred Strong clinical judgment, communication, and collaboration skills Commitment to patient-centered care and ability to travel between facilities as needed
