Job Description

Internal Medicine / Family Medicine Locums - SNF/LTC | Montana Locums coverage is needed in Montana for a geriatric skilled nursing facility/long-term care population. This is a 3-day-per-week, daytime opportunity with no call and dedicated APP support. Highlights Tuesday-Thursday preferred 7:00 AM-5:00 PM , 10-hour onsite shifts No call - hospitalists cover overnight call Average 12-14 patients per shift Dedicated APP support with physician supervision Geriatric and medically complex patient population Responsibilities Provide comprehensive medical oversight for SNF rehab and LTC residents, including: SNF admissions and H&Ps Routine/chronic and acute care Regulatory and recertification visits Medication management Care planning and interdisciplinary collaboration Family/goals-of-care discussions Transitions of care and palliative/comfort-focused care Required documentation and regulatory compliance Requirements MD or
DO BC/BE
in Internal Medicine or Family Medicine SNF, LTC, post-acute, transitional, or geriatric experience ACLS and BLS Montana license required; active
IMLC LOQ
