Northern New Hampshire Primary Care Physician Job Clinical Role:

Seeking a board certified or truly board eligible Primary Care PhysicianPermanent job opportunityWe are seeking a dedicated and compassionate Family or Internal Medicine Physician. This is an opportunity to join our team in a rural community in northern New Hampshire. This role offers the opportunity to make a meaningful impact by providing comprehensive care to patients of all ages (or adults) in a close-knit, underserved community. Rural medicine allows the delivery of high-quality primary care, including preventive, acute, and chronic disease management, to participate in community health initiatives, and promote wellness educationProvide comprehensive medical care including physical exams, diagnoses, and treatment plans; develop personalized health plans for patientsManage long-term treatment plans for patients with chronic illness, ensuring regular follow-ups and monitoring; manage acute care and chronic medical conditions and commoditiesOffer preventative care services such as immunizations, screenings (eg, cancer, cholesterol, blood pressure), and well-ness check-ups. Order diagnostic testing as appropriate; follow-up with patients on test results, treatments, and overall care plansPrescribe medications and treatments in accordance with best practices and patient needs; work closely with patients to adjust medications and treatment strategies as neededCoordinate with specialties for referrals when necessary to determine a course of treatmentEducate patients on disease prevention, nutrition, lifestyle changes, and managing health conditions; provide patients and families with information on health maintenance, disease management, and healthy livingCollaborate with nurses, medical assistants, and other healthcare providers to ensure continuity of care; assists support staff in improving their performance be participating in monitoring and evaluating of the quality and appropriateness of patient careMaintain medical records using the current format and completion within the timeframes specified be the Medical Staff Bylaws and PoliciesProvide excellent medical care to patients by maintaining a level of professional practice and conduct commensurate with community professional standards as required for continued participation in and reappointment to the Medical StaffParticipate in quality improvement initiates within the practiceBoard certified (or eligible) by WMC Medical Staff BylawsACLS and BLS from the American Heart Association requiredSuccessful completion of an approved residency by the Accreditation Council for Graduate Medical Education, the American Osteopathic Association, or the Royal College of Physicians and Surgeons of CanadaMust be comfortable having own patient panel and practicing autonomouslyHigh elderly populationExperienced in Chronic Disease Management16-22 patients per day36 patient hours, 4hrs of admin timeMay participate in day-time call rotation (no after hour call)EMR is Meditech Expanse, Dragon dictationSupported by an RN Team leader and medical assistant Allow RecruitWell to set up a phone call with you and the client to discuss more!
