Hospitalist Physician - Post-Acute & Long-Term Care Part-Time / Per Diem | Flexible Schedule | $140/hourNorth Ocean Medical Group | Suffolk County, Long Island, NY North Ocean Medical Group is seeking a Hospitalist Physician to join our growing post-acute and long-term care program serving skilled nursing, rehabilitation, and assisted living communities across Suffolk County. This position is specifically designed for a practicing hospitalist who would like consistent, flexible clinical work outside of their hospital schedule while remaining part of an established physician-led medical group. Hospitalists working 7-on/7-off or other block schedules are strongly encouraged to apply. The Opportunity The physician will provide scheduled on-site medical care within skilled nursing, rehabilitation, long-term care, and assisted living settings. Our goal is to bring hospital-level clinical experience into the post-acute setting — providing timely evaluation of medically complex residents, managing changes in condition, supporting transitions from hospital to facility, and helping reduce avoidable emergency department transfers and rehospitalizations. The position will begin on a part-time/per-diem basis, initially approximately one scheduled rounding day per week depending on facility needs, with the opportunity to add additional days as North Ocean Medical Group's facility program expands. Responsibilities Provide scheduled on-site physician rounding at assigned facilities Perform new admissions, post-hospital evaluations, and follow-up visits Manage acute changes in condition and chronic medical illnesses Care for medically complex and geriatric patients Evaluate residents when hospitalization or emergency department transfer is being considered Assist with hospital-to-SNF transitions of care Collaborate closely with nursing staff, advanced practice providers, rehabilitation teams, specialists, and facility administration Communicate effectively with patients and families Complete timely and appropriate clinical documentation Participate in physician coverage arrangements as mutually agreed upon Ideal Candidate Currently practicing or recently practicing as a hospitalist Board-Certified or Board-Eligible in Internal Medicine Unrestricted New York State medical license or eligibility for licensure Strong clinical judgment in managing medically complex patients Comfortable working with geriatric and post-acute populations Responsive and collaborative with nursing teams Reliable, professional, and comfortable practicing independently Interested in developing a consistent, long-term relationship with North Ocean Medical Group Previous skilled nursing or long-term care experience is helpful but not required. Hospital medicine experience is the primary clinical background we are seeking. Schedule Part-Time / Per Diem Flexible weekday scheduling designed to complement an existing hospitalist schedule. This opportunity may be particularly attractive to physicians working 7-on/7-off, 5-on/5-off, or similar block schedules. Initial commitment may be approximately one rounding day per week, with opportunities for additional clinical days and facility assignments as our program grows. Compensation $140 per hour Scheduled clinical time with predictable part-time/per-diem compensation. Why North Ocean Medical Group?
