As a Skilled Nursing Facility (SNF) Physician, you will oversee medical care for Participants in SNFs, perform assessments, manage acute and chronic conditions, and coordinate with interdisciplinary teams to ensure seamless care transitions. Responsibilities include rounding in SNFs, developing care plans, facilitating hospital discharges, and participating in daily team meetings. On-call coverage is shared, with a focus on reducing hospitalizations and promoting quality of life. Ideal candidates are mission-driven, collaborative, experienced in geriatrics or skilled nursing, and possess strong communication skills. Requirements include an active PA medical license, board certification in Internal Medicine or Family Medicine, and DEA registration.
