Accomplished Physician Needed for Comprehensive Senior Care Center - Uniontown/Greensburg Monday-Friday 8am-430pm Senior LIFE is an innovative home and community based Medicare and Medicaid funded program which provides all-inclusive healthcare services and support to seniors living in the community. Senior LIFE makes it possible for seniors to remain at home, enjoying the comfort and reassurance of familiar surroundings while receiving the care they need. Senior LIFE staff live the mission of the program. Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services. The Senior LIFE physician will be expected to perform standard medical tasks throughout the day, working with elderly members and addressing a host of common and atypical diagnoses. Additionally, the physician will serve as a critical member of multiple interdisciplinary teams, working in tandem with social workers, dieticians, occupational/physical/speech therapists and other caregivers to ensure the most cutting-edge and comprehensive care available.
Daily responsibilities include:
Performing comprehensive histories and physicals on new referrals to determine the best course of care, while setting member-specific goals based on patient wishes, actions and prognosis Performing interval histories and physicals on existing members, completed in accordance with approved care schedules or as needed due to changes in medical status Evaluating and treatment episodic illnesses, and working with other interdisciplinary team members to ensure proper care in all areas Making specialist referrals for medical needs beyond the immediate scope of expertise, or when a second opinion is required/requested Admitting and/or co-managing members in the hospital, providing primary care/attending support in conjunction with the facility's resident physicians/specialists Coordinating discharge plans for members leaving hospitals or other specialized care facilities, working in tandem with LIFE Center therapists, dieticians, transportation professionals and medical staff Effectively communicating with Senior LIFE staff, members and external family/caregivers to determine extended treatment plans and educate stakeholders accordingly This role requires a versatile, passionate physician with deep knowledge of and compassion for the elderly. The position is very unique within the medical community, from its interdisciplinary approach to member care to the innovative, integrated approach practiced at all centers. The LIFE Center physician will have significant medical oversight and, moreover, the ability to change existing notions of eldercare in America—and, as such, Senior LIFE seeks a truly pioneering leader. All applicants should be graduates of accredited medical schools with a valid license to practice medicine in Pennsylvania. A fellowship in geriatrics is preferred. Candidates should have a minimum of one year's experience working specifically with a frail/elderly population, plus proven clinical leadership skills and the desire to work in a fast paced team environment.
