Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Role:
We are seeking Physicians specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. A valid medical license to practice and prescribe legally in any US state is required, and eligibility to obtain additional state licenses as needed is required. This role is designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in working collaboratively with Endocrine Advanced Practice Providers (APPs) in providing high-quality patient care. A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy. This is a fully remote W-2 position (US-based), with flexible scheduling options: 32 total hours per week, 36 total hours per week, or 40 total hours per week. From the total working hours, 80% will be clinical/patient-facing care, and 20% administrative time.
What You Will Do:
Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs) The role could involve having your own panel of patients, or collaborating with APPs and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes. This role could involve APP collaboration and supervision, as needed Review charts and conduct initial and return visits to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols. Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program
About You:
Must have an MD or DO degree
Required:
Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
Highly Preferred:
Certification by the American Board of Obesity Medicine (ABOM) 2 years of clinical experience and pharmacotherapy (older generation oral and newer injectable medications) in obesity medicine or endocrinology, post residency OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine, in lieu of years of experience
Required:
Active and unrestricted Medical State License in any US State, and eligibility to obtain additional state licenses (IMLC preferred, but not required) Must be currently opted-in with Medicare Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens Interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes, as needed Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity Open to working one of the following W2 roles: 32hrs/week, 36hrs/week or 40hrs/week Committed to delivering patient-centered, culturally competent care Excellent communication and interpersonal skills Experience providing synchronous patient care in a telehealth setting is preferred Bilingual candidates with medical fluency in Spanish are desired, but not required Form Health's benefits: Competitive salary Comprehensive health benefits.
