About Us Aviator Health supports older adults and patients with chronic conditions. We are actively recruiting a Remote Telehealth Psychiatrist. In this position, you will conduct audio patient consultation appointments to understand the challenges that the patient is facing. You will determine if a patient is suited to receive patient advocacy under the principals of Principal Illness Navigation and/or Community Health Integration. This role does not require diagnosing or prescribing. Position Overview We are on a mission to give patients the care they deserve by making it easy to navigate the healthcare system. American healthcare is so complex that 88% of Americans do not have the health literacy necessary to navigate the system; this is especially true for patients managing behavioral health needs alongside chronic conditions. Navigating healthcare today is overwhelming, especially for patients living with chronic conditions and unmet social needs. At Aviator, we're building a new model of support that combines human expertise with technology to make care more accessible and coordinated. Psychiatrists are central to this effort, helping ensure patients' behavioral health needs are identified, addressed, and connected to the right level of care. Responsibilities - Remote Telehealth Psychiatrist Conduct patient consultations via phone to assess the health challenges patients are facing. Determine patient suitability for advocacy support under Principal Illness Navigation (PIN) and/or Community Health Integration (CHI) care models. Partner with Health Advocates to build and oversee holistic care plans. Identify and prioritize patient needs, ensuring continuity of care. Integrate social determinants of health (e.g., food, transportation, housing) into patient care plans. Use technology and AI-enabled systems to deliver care efficiently and effectively. Collaborate with Aviator's technology team to develop innovative AI tools that enhance the clinical experience.
