This role offers an exciting opportunity to deliver comprehensive patient Evaluation and Management assessments, perform necessary procedures, and collaborate with multidisciplinary teams to ensure optimal health outcomes. The Visiting Doctor will bring a proactive approach to patient care, utilizing their extensive medical expertise to serve diverse populations in home and residences, institutional facilties in various community environments. This position is ideal for a motivated healthcare professional eager to make a meaningful impact through clinical excellence and innovative practices. Responsibilities Conduct thorough patient evaluations, including physical examinations, vital signs assessment, and detailed medical histories. Manage complex and simple clinical orders and evaluation for patients including medications, home health services, facility admission, hospice care and personal care and other management techniques. Manage acute and chronic conditions in specific settings, isuch as home, assisted living facilties, independent living facilties, group homes and daycare centers. Collaborate with healthcare teams on diagnostic evaluations utilizing medical imaging, laboratory data collection, and EMR systems like practice fusion and claim MD. Provide telehealth consultations, ensuring seamless remote patient assessments and follow-up care. Assist in discharge planning, hospice & palliative medicine, and post-acute care coordination to support patient recovery at home or in specialized facilities. If necessary, supervision and collaboration agreements of mid-levels for the continuance of care for patients at home. Experience Proven general and primary care experience with exposure in various level of E/M and clinical care experience . Demonstrated proficiency in clinical charting for specific E/M at home and institutionel facilties. Familiarity with EMR systems including practice fusion or similar platforms; experience with CPT coding and ICD coding is advantageous. Background in r primary care settings with a focus on patient assessment and acute pain management. Knowledge of medical terminology, anatomy knowledge, aseptic techniques, and diagnostic evaluation methods essential for delivering high-quality patient care.
