Job Description

Family Medicine Physician | Jefferson Job scope: Provides comprehensive primary care to patients of all ages in a clinic setting. Responsibilities include diagnosing and treating acute and chronic conditions, performing preventive care, and managing overall patient health through regular checkups and screenings." Serves as a medical team member of Core Health Systems. Will coordinate care with local hospital systems, FQHC, and LMHA.
Education:
Graduate of an accredited Medical School. Completion of accredited Residency program. Board certification or board eligible.
Licensure:
Current licensure as an MD/DO to practice in the State of Texas by the State Board of Medical Examiners. Current
DEA, DPS
certification.
Position requirements:
Must be able to meet and maintain current CHC credentialing and privileging requirements. Ability to supervise APRN/PA in practice in the primary care/integrated clinic settings.
Experience/Knowledge:
3-5 years experience preferred. Ability to enhance a positive team working relationships Physical requirements that are bona fide occupational qualifications essential to the performance of the job: Ability and strength to sit, bend, and reach during an 8to 10 hour day. Able to lift a minimum of 10 pounds. Sensory acuity sufficient to evaluate, treats, and monitors patient needs and maintains accurate records. Fine motor skills sufficient to legibly document and communicate. Routine 8to 10 hour shifts with weekends off.
