Job Description

Outpatient Internal Medicine / Family Medicine Physician — 4-Day Week | No Call, No Weekends, No Cor Synergy Weight Loss Lewisville, TX Job Details Full-time $144,000 - $230,000 a year 22 hours ago Benefits Disability insurance Health insurance Dental insurance 401(k) Paid time off Parental leave Vision insurance 401(k) matching Flexible schedule Life insurance Malpractice insurance Qualifications Outpatient experience Telehealth therapy Internal medicine Patient interaction Family medicine Full Job Description Synergy is a physician-owned, independent primary care and obesity medicine practice in Lewisville, Texas. We are owned and run by two people — a practicing physician and our CEO. There is no health system above us, no private equity behind us, and no regional VP whose opinion outranks yours about your own patient. We built this practice because we wanted to see what medicine feels like when you strip out everything that isn't medicine. What We Actually Removed No annual compliance module marathon. No 40-slide modules on topics you already know, due at midnight. No corporate badge, no six logins. One EHR. You open a chart and you are in it. No RVU dashboard. No productivity huddle, no color-coded scorecard, no "opportunity" conversations about your panel throughput. No inbox designed by committee. Messages route to staff first, not to you. No call. No weekends. No hospital rounding. No nursing home coverage. Every decision about how you practice is made by two people, one of whom is a physician who still sees patients. If something is stupid, we can change it that week. Compensation $12,000/month guaranteed floor — from day one, permanent. Not a draw. Not a ramp-year gimmick that expires. 40% of your net collections. You are paid the greater of the two, every single month. As your panel fills, the floor stops binding and the percentage takes over. 12% recurring royalty on the monthly service charge for every weight-loss subscription patient you originate — paid monthly, for as long as that patient stays with us. This is real recurring income that compounds with tenure, and it is unusual for an employed physician role. Signing bonus for the right candidate. No RVU targets. No quality-metric clawbacks. No withhold. You see patients, we collect, you get paid on what actually came in the door. Schedule Four-day clinical week 18-patient daily cap — this is a ceiling we enforce, not a target we nudge you past Longer appointment slots for complex and metabolic patients Three weeks of paid time off (12 clinical days), accruing monthly, bankable up to 24 days Flexibility on which four days, and on building a partial telehealth block if that suits your life Benefits Employer-paid malpractice Medical license renewal and DEA registration reimbursed Short-term disability coverage 401(k) with employer contributions that grow with your tenure here Dedicated private physician office Who We're Looking For Board Certified or Board Eligible in Internal Medicine or Family Medicine Licensed in Texas, or eligible for Texas licensure Genuine interest in metabolic health, obesity medicine, and GLP-1 therapy — or willingness to learn it properly (we will train you and support ABOM certification) Comfortable owning a panel and building long-term patient relationships Wants to be part of building something rather than maintaining someone else's machine Where This Goes We are an early-stage practice with a growing panel and a second location in planning. The physician who joins now is not filling a slot — they are the first associate physician in the group, with a path to Associate Medical Director and first right of refusal on the medical directorship of our next location. If you want a defined seat in a large organization, we are honestly not the right fit. If you want a four-day week, a real cap on your day, recurring income you build yourself, and a practice where nobody makes you click through a module about workplace ergonomics — we should talk.
