Job Description

inFORME healthcare is looking for a dynamic, clinically adept Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our Physician leaders at in a post acute hospital. This is a unique dual hospital location. Your role will be to manage inpatients that have a higher acuity in the Sheboygan, WI area. The patient diagnostic complexity involves managing trach/vent/complicated wound and multiple co-morbidities. You must be comfortable managing acute changes in place. This position would involve working a 7 on 7 off shift schedule (10 hours on site then call off site for the remainder of the 12 hours shift) and sharing the caseload with a physician daily. This will include weekend rotations to manage patients and collaborate with our Hospitalists. Additionally, you would collaborate with corporate leadership to assist in maintaining optimal momentum of our stellar quality metrics, program components and implementing our proprietary protocols to achieve clinical outcomes. We are seeking a dynamic and talented NP/PA to join our team! If you are driven by achieving a high standard of clinical excellence and enjoy an acute care collaborative environment, this position is a great match for you! Experience - Minimum 3 to 5 years of acute inpatient hospital experience as a physician assistant or a nurse practitioner is required and ability to work autonomously. Education - Educational background consistent with requirements for licensure and certification with the State of Wisconsin.
