Mon - Fri, 9:00 AM - 5:00 PM (with on-call rotation)

We are seeking a dedicated Physician to provide medical care for patients in Long-Term Care (LTC) and Transitional Care Units (TCUs) in the Hudson/St. Croix Falls, WI area. The provider will ensure high-quality, patient-centered care for residents in long-term and post-acute care settings.
Key Responsibilities :
Perform comprehensive initial and ongoing assessments of LTC and TCU patients. Develop and implement individualized care plans based on medical history and patient needs. Provide routine and urgent medical care, including diagnosing and treating acute and chronic conditions . Prescribe medications, order diagnostic tests , and adjust treatment plans accordingly. Collaborate with an interdisciplinary team (nurses, therapists, social workers) to optimize patient care. Educate patients, families, and caregivers on treatment options, medication adherence, and disease management . Ensure documentation is accurate and compliant with regulatory standards. Address end-of-life care decisions and collaborate with palliative and hospice teams when needed.
Qualifications :
MD or DO degree from an accredited medical school. Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred. Experience in long-term care, transitional care, or geriatrics is preferred but not required. New Graduates are Accepted Valid medical license to practice in Wisconsin.
