West Michigan - including Grand Rapids and surrounding communities HomeMD Housecall Services is currently transitioning to a new name and will soon be known as HomeNP Health Solutions . While our name is evolving, our commitment to delivering compassionate, patient-centered care remains the same. We are seeking a Podiatry-Focused Nurse Practitioner (NP), Physician Assistant (PA), or Podiatrist (DPM) to support the healthcare needs of older adults throughout the West Michigan region. This role focuses on routine foot care, diabetic foot management, skin and wound monitoring, and preventative care in the home setting. No surgical procedures are required. Our providers deliver care directly to patients in their residences, assisted living communities, and other residential care settings. This model allows us to improve access to healthcare, reduce barriers to treatment, and build meaningful relationships with the seniors we serve. The ideal candidate has a passion for geriatric care, strong clinical judgment, and an interest or background in podiatry-focused services. Providers work independently in the field while receiving support from our clinical, scheduling, and administrative teams. About the Role You will play an important role in promoting patient wellness by conducting assessments, monitoring ongoing conditions, and identifying concerns before they become more serious. Through patient education and preventative care, you'll help improve quality of life and support aging in place safely. This opportunity offers autonomy, flexibility, and the ability to make a direct impact within your local community. Primary Responsibilities Conduct comprehensive patient evaluations and review medical histories Assess foot health, skin integrity, circulation, and mobility concerns Monitor and manage diabetic foot conditions and other lower extremity concerns Identify podiatry- and wound-related issues and develop appropriate care plans Order, interpret, and follow up on diagnostic testing as clinically indicated Prescribe medications and therapies within scope of practice Coordinate care with primary care providers, specialists, caregivers, and facility staff Maintain accurate and timely documentation of patient encounters Educate patients and caregivers regarding conditions, medications, and preventative care Encourage safe and healthy practices within the home environment Support early intervention efforts to reduce complications and hospitalizations Preferred Experience Experience in one or more of the following areas is highly preferred: Geriatrics Podiatry-focused care Diabetic foot management Wound care Home health Primary care Long-term care or assisted living settings Chronic disease management Qualifications Nurse Practitioner (NP) Active Michigan Nurse Practitioner license Master's or Doctoral degree in Nursing Current national board certification Experience or strong interest in podiatry-focused and geriatric care Physician Assistant (PA) Active Michigan Physician Assistant license Master's degree from an accredited PA program NCCPA certification preferred Experience or strong interest in podiatry-focused and geriatric care Podiatrist (DPM) Active Michigan Podiatric Medicine license Experience providing foot care services to geriatric populations preferred What We're Looking For Strong understanding of patient care guidelines and regulatory compliance Excellent communication and interpersonal skills Professional integrity, reliability, and accountability Ability to work independently and manage a mobile schedule effectively Compassionate, patient-centered approach to care Commitment to continuous learning and professional development Why Join HomeMD / HomeNP Health Solutions?
