Job Description

LCSW Qualifications 1. Is a clinical social worker licensed by the California Board of Behavioral Science Examiners and works in consultation with a psychologist or board certified psychiatrist. 2. ADHC preferred but not required. 3. Spanish speaking preferred but not required. Duties 1. Provide consultation services to the center staff for a minimum of three hours each month and documenting specifically which staff received the consultation and the nature of the consultation services. 2. Provide consultation services to participants when a. the participant's diagnosis or medical history indicate a mental, emotional or behavioral problem. b. information from the participant's family indicated mental, emotional or behavioral problems may exist. c. the psychosocial assessment developed by the social worker indicates apparent mental, emotional or behavioral problems that need further assessment by the consultant. d. observation by the center staff indicates the need for an assessment by the consultant./ 3. Attend and participant in MDT meetings as needed 4. Conduct an initial assessment of any participant referred for evaluation and every six months if psychiatric/psychological services are being provided 5. Prepare an individual plan of care for a participant if psychiatric/psychological services are being provided 6. Act as the liaison with other members of the MDT, family members, etc that may yield information for or assist with the provision of psychiatric/psychological services. 7. Other duties as assigned by
