Job Description

The LMHP is responsible for all clinical services and supervision of all non-licensed staff. The LMHP functions include, but are not limited to gathering of data for the assessment, conducting, at a minimum, one face to face interview with the recipient and their family/significant others as well as sign and date the assessment document, administering and scoring LOCUS/CALOCUS, if an Approved Clinical Evaluator (ACE), and acting as team leader of the service planning team.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following: Assures Active Recipient Involvement Provides identified psychotherapy services to recipient Provides aggressive outreach if the recipient begins to participate less than specified Assures continuity of care when recipient is placed in an institutional setting, such as the hospital, jail, etc. Serves as team leader for the MHR assessment and completes the strengths assessment and integrated summary. Develops Treatment Plan and prepares subsequent updates in conjunction with the team of service providers Assures development of goals, objectives, and interventions that are consistent with the Treatment Plan Provides oversight for the integrated implementation of service goals, objectives, and interventions Completes the
LOCUS/CALOCUS
Scoring Sheets in a timely manner Assures appropriate, effective service delivery Convenes the Service Planning Team, as necessary, to adjust the Treatment Plan to the needs of the recipient Must attend all mandated meetings conducted by the Bureau (DHH) or its designee
