Job Description

H2Health is seeking an Outpatient Hand Therapist CHT to join our patient-centered rehabilitation team. In this role, you will evaluate and treat patients with hand and upper extremity conditions, creating individualized, evidence-based treatment plans to restore function and improve quality of life. You'll collaborate closely with physicians and a multidisciplinary therapy team, provide patient and family education, and document care in our EMR. Responsibilities Provide outpatient hand therapy evaluations and develop individualized treatment plans Deliver evidence-based interventions for upper extremity injuries and post-surgical cases Collaborate with physicians, therapists, and clinic staff to coordinate care Educate patients and families on home exercise programs and injury prevention Document treatments, progress notes, and outcomes in the EMR system Participate in mentorship, training, and continuing education opportunities Contribute to a collaborative, patient-centered clinic culture Support quality improvement and best-practice initiatives across clinics Required Skills Hand therapy evaluation and treatment planning Upper extremity rehabilitation Post-surgical rehabilitation Orthotic/splint fabrication and fitting Evidence-based treatment techniques Electronic medical records (EMR) documentation Patient and family education Interdisciplinary collaboration Outcome measurement and tracking Time management and caseload organization
