Activity Assistant Be Part of Our Team:
Full-Time About Us:
Santana Hills Healthcare Center is a 167-bed facility with 90 skilled nursing beds and 77 sub-acute beds, delivering compassionate, patient-centered care for rapid recovery in a central San Jose location. Activity Assistant - Skilled Nursing Facility Key Responsibilities Assist in planning, organizing, and implementing recreational and therapeutic activities for residents. Encourage resident participation in group and individual activities. Support residents during activities, outings, and special events. Help set up, decorate, and clean activity areas before and after programs. Maintain activity supplies, equipment, and inventory. Document resident attendance, participation, and responses to activities. Assist with transporting residents to and from activity locations as needed. Build positive relationships with residents and promote social engagement. Follow resident care plans and accommodate physical, cognitive, and emotional needs. Ensure activities comply with facility policies, safety standards, and regulations. Collaborate with the Activities Director and interdisciplinary care team. Assist with holiday celebrations, community events, and family activities. Qualifications High school diploma or equivalent preferred. Experience working with seniors, long-term care, or healthcare settings preferred. Strong communication and interpersonal skills. Compassionate, patient, and enthusiastic attitude. Ability to work effectively with elderly residents, including those with dementia or cognitive impairments. Ability to lift, push, or transport activity materials and assist residents when necessary. Basic computer and documentation skills. CPR/First Aid certification preferred (if required by facility). Desired Skills Creativity in developing engaging activities. Teamwork and collaboration. Organizational and time-management skills. Ability to motivate and engage residents. Flexibility to work weekends, holidays, or special events as needed.
