Make a Difference Every Day Westview Manor of Peabody is looking for a compassionate, organized, and energetic professional to serve as our Social Services & Activities Director . This unique leadership role combines social services with therapeutic recreation in a mental health-focused long-term care facility . Unlike a traditional nursing home, many of our residents are independent with their daily living activities. Mental health-focused long-term care Smaller facility with a family atmosphere Residents work toward independence and community integration Supportive interdisciplinary team Opportunity to make a lasting difference in residents' lives Leadership that values innovation and employee input Position Summary The Social Services & Activities Director is responsible for overseeing the facility's psychosocial services while developing and coordinating therapeutic recreational programming that promotes residents' emotional, social, cognitive, and physical well-being. This position works closely with Nursing, Prairie View Mental Health, Therapy, Guardians, Families, Case Managers, and the Interdisciplinary Team to ensure residents receive individualized services that support recovery, independence, and person-centered care. Responsibilities Social Services Complete admission, quarterly, annual, and significant change psychosocial assessments Develop and update individualized care plans Participate in interdisciplinary care conferences Assist with discharge planning and community placement Coordinate resident appointments and referrals Maintain communication with guardians, families, case managers, and outside providers Assist residents with grievances and advocate for resident rights Provide crisis intervention and emotional support as needed Complete required documentation in PointClickCare (PCC) Ensure compliance with KDADS, CMS, and facility regulations Activities Plan, organize, and implement engaging therapeutic activities Develop monthly activity calendars Coordinate community outings and special events Encourage resident participation through individualized programming Document participation and progress Supervise activity staff and volunteers (if applicable) Order activity supplies and manage department budget Create programs that support socialization, coping skills, independent living skills, and recovery
