Rehabilitation-PT Activities Coordinator - •Per Diem•
IMPORTANT NOTE
In lieu of benefits due to "per diem" status, 15% will be added to the hourly rate. Per diem employees are offered work on an "as-needed" basis.
POSITION SUMMARY
The Activities Coordinator will be responsible for the implementation of the activity program by planning and reviewing the residents activity calendar; this includes assessing interest, response and outcomes documented in the progress notes.
POSITION REQUIREMENTS
Minimum Education:
High School graduate or equivalent.
Minimum Qualifications:
Must have working-level knowledge of the English language, including reading, writing and speaking English.

Two years of experience working in skilled nursing facility and/or assisted living. Certification(s):

State and National Certification of Activities Professionals.

American Heart Association approved CPR/BLS required.
Other:
Valid NV/CA drivers license.
SkillsComputer/Typing:
Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.

Proficient in MS Office.

Essential Physical RequirementsIt is common to encounter potential hazards in the healthcare environment. Some of these hazards could include, but are not limited to: radiation, toxic chemicals, biological hazards, heat, cold, noise, dust and stress. Carson Valley medical Center has a Safety Management program in place addressing these issues.
POSITION ESSENTIAL FUNCTIONS
Activity ProgramResponsible for assessment, planning, implementation and evaluation of activity programs for swing bed rehabilitation. In the provision of these services, the activities coordinator will consult and collaborate with other members of the interdisciplinary team. The activity coordinator helps to identify community partners to ensure growth and development of recreational and community integration program to improve residents lives during and after rehabilitation. This position reports directly to the Manager of Rehabilitation Therapies.
Nature and Scope:
the activity coordinator must provide optimal patient care through assessment, planning, implementation and evaluation of adult and geriatric residents and families. This position conducts evaluations, and develops an appropriate activities program from admission through time of discharge from the facility. At times, this position may be asked to participate in program development and outcome measurements in the clinical setting. Responsible for the day to day implementation of the Swing Bed Rehab Activity Program by planning and reviewing the daily and weekly activity calendar; this includes assessing attendance and outcomes documented in the progress notes. CommunicationCommunicates with Occupational Therapist or Rehabilitation Services Manager on a regular basis to update response to activities by the patient/resident.
