Job Duties What You'll do: Participates directly or indirectly in the development of the Individual Treatment Plan (ITP) for patients (e.g., provides input on the plans designed around the habilitation and rehabilitation in cooperation with the multidisciplinary treatment team) Recommends and works in collaboration with treatment team on changes in treatment plans as behavior shows progress or regression and carries out patient or group programming delineated from the multidisciplinary treatment team Trains patients to function independently in self-help and daily living skills Completes activity of daily living for those unable to perform tasks (e.g., hygiene, dressing, feeding/eating, grooming, moving/lifting patients and toileting to include application/changing of incontinence pads/briefs and bedding Provides preventative skin care and physical shower assistance Measures height/weight Assists in social skills such as writing/written communications, identifying name and addresses, budgeting, and time telling by using a variety of training techniques Directs and monitors vocational/educational responsibilities Assists in escorting patients and/or accompanies to scheduled meetings (e.g., nurse sick call, doctor's sick call, education and job related) Performs specific tasks with patient as assigned by supervisor and/or multidisciplinary treatment team (e.g., vocational and programming task) Assists in the planning of and implementation of leisure/recreational activities (e.g., games, gardening, library visits, recreational yard/activities, commissary, religious programming, community service) and documents the activities and outcomes in progress notes. Institutes appropriate crisis intervention for unacceptable behavior as defined by treatment plan and prevents injury of patient to self or others Completes necessary documentation of incident reports as required Monitors on and off unit surveillance of patient behavior Investigates patient noncompliance with unit rules May assist in disciplinary process Responds to emergencies and recognizes unusual and threatening conditions and takes appropriate action Maintains safety, security and sanitation of patient and living area per Post Orders to include quality unit rounds Reports and documents any problems and unusual incidents directly to program and institutional staff Maintains control of own emotional response to prevent escalating aggressive behavior and direct monitoring of patient Assists with shakedowns of patient cells and unit area for contraband, dangerous or unusual objects Provides orientation to new patient arrivals Provides training, directions and/or feedback to patients regarding behavior observed Completes charting and documentation in Subjective, Objective, Assessment, Plan (SOAP) format or electronic equivalent Records observation logs, activities, completes forms and reports that are related to shift attendant's duties and unit operations Reviews shift report from preceding shift(s) and post orders Acts as patient representative or advocate as assigned Attends meetings (e.g., treatment team/level meetings) and serves on institutional committees Attends in-service training and educational programs Provides coverage in the absence of coworkers Why Work for the State of Ohio At the State of Ohio, we take care of the team that cares for Ohioans. Pre-Employment Drug Testing:

All final applicants tentatively selected for this position will be required to submit to urinalysis to test for illegal drug use prior to appointments.
