Pay:

$51.00 - $55.75 per hour based on experience

Type:

Three-month contract

Schedule:

32 hours guaranteed, 8:00AM - 4:30 (Monday - Friday) On Call /

Weekend Requirement:

Ability to take back up call one weekend a month and one night of the week.

Coverage Areas:

Marshall, Jackson County Alabama

Primary Responsibilities:

• The coverage area is close to 40 miles from our office.
• Come to the office for IDG every other Wednesday.
• Ability to do IDG care team meetings as well as notes to prepare for meets just like case manager.
• Seeing up to six patients per day and documentation.
• Ability to do admission visits, recertifications and death/discharges.
• Ability to work independently and take patient medications, ordering supplies, ensuring all their needs are met and arefortable.
• Manage home hospice patients.

Qualifications:

• Current RN License in Alabama
• Experience in homecare preferably as a Case Manager
• Home hospice experience
• Ability to work independently
• Valid Driver's License
