Qualifications & Experience:
Active RN license in good standing Demonstrated experience in rehab or long-term care settings Strong clinical assessment and critical thinking skills Compassionate approach toward patients and families Ability to collaborate within a multidisciplinary healthcare team Excellent communication and organizational abilities
Primary Responsibilities:
Provide comprehensive nursing care to residents in a rehabilitation/long-term care unit Monitor, assess, and document patients' health status and progress Administer medications and perform treatments in adherence to facility protocols Develop, implement, and update individualized care plans in collaboration with physicians and allied health professionals Educate residents and family members on treatment plans and health maintenance Ensure compliance with regulatory standards and facility policies Respond promptly to changes in residents' conditions and coordinate appropriate interventions
