Job Description

Overview Department Overview The Nurse Family Partnership under our subsidiary is a program that provides comprehensive services to first time mothers and their newborn child. This program has the ability to improve the lives of the families and children experiencing the effects of poverty and adversity. Our specially trained nurses work closely with the mother and child to assist them through this stage in their life. Primary Responsibilities Our subsidiary Nurse Home Visitor is responsible for making home visits to mothers enrolled in the Nurse Family Partnership (NFP) program. This position works with the mother and her child while eligible for services under the program. These services include providing care coordination plus working with the mother and the child to provide some basic services such as blood pressure monitoring and weighing the child. Case documentation is recorded to document the visits for billing and record of services. This position will be required to network with community resources and the medical community to integrate our program into the right resources in local communities. This position must be able to interact with and direct the efforts of external stakeholders in order to be successful. Summary of Qualifications Bachelor of Science in Nursing or Associate Degree in Nursing Active Alabama license in good standing as a Registered Nurse (RN)
BLS CPR/AED
Certification Experience in a position exercising independent judgment and decision making Experience in a position demonstrating exceptional written, verbal, and interpersonal communications Daily travel as required for the job Valid Alabama Driver's License Experience working in public/community health, including home visitation preferred Experience in a position providing maternal/child health services, particularly in community setting preferred Experience working with diverse populations, communities, and organizations preferred Terms and Agreements By submitting a job application, I attest that all information to the best of my knowledge is true and accurate.
