SouthernCare, a Gentiva company, is seeking a Hospice Registered Nurse to provide compassionate end-of-life care to patients in the Tuscaloosa area. In this role, you will manage a caseload of hospice patients, conduct comprehensive assessments, develop and update individualized care plans, and administer medications and treatments focused on comfort and symptom management. You will coordinate closely with physicians, social workers, chaplains, and aides, ensuring seamless, patient-centered care. The Hospice RN also educates and supports families, documents care accurately, and participates in on-call rotation to maintain high-quality, responsive hospice services.
